After your Bariatric Evaluation Consent and Payment Package has been received and your payment processed, you will receive another secure invitation to access your Digital Bariatric Evaluation Intake Packet.

Once your completed intake packet has been received and reviewed for completeness, you will receive your final secure link to schedule your 30-minute telehealth consultation at a time that is convenient for you.

Bariatric psychological evaluations are currently available to clients who will be physically located in New York, Oregon, or Pennsylvania at the time of their telehealth evaluation.
